Overview
Job Description: The P&BH Care Navigator will be responsible for providing care and guidance to high-risk patients to promote better health outcomes. The position will work to eliminate barriers to care by guiding, educating, and navigating patients to receive necessary health care monitoring and resources, both within Centra and externally. The position will be charged with building professional relationships with the patient and their families, local and regional healthcare providers to ensure smooth transitions and timely access to care for patients. The P&BH Care Navigator is expected to have extensive knowledge of local resources on order to successfully link patients and families to the appropriate Behavioral Health resources and supports based on their specific needs and insurance status.
The P&BH Care Navigator role supports The Virginia Mental Health Access Program (VMAP) collaboration with Centra Psychiatry & Behavioral Health (P&BH)
VMAP is a statewide initiative that helps health care providers take better care of children and adolescents with mental health conditions through provider education and increasing access to child psychiatrists, psychologists, social workers, and care navigators. VMAP ensures that more children have access to providers who are better able to screen, diagnose, manage, and treat mental health.
Centra partners with VMAP by employing and engaging Child/Adolescent Psychiatrists, Licensed Mental Health Professionals, and Care Navigators to meet regional Child/Adolescent Psychiatry & Behavioral Health needs.
Remote Care Navigator position in Psychiatry and Behavioral Health for the Virginia Mental Health Access Program which facilitates mental health access for children and adolescents.
